Hooray! You’ve found a spot at Birch Tree Academy. Now it’s time for the fun part: filling out all kinds of forms. Seriously though, we know it’s a drag and we don’t ask because we love it—Washington State requires that we collect this information for each student. Please select your school below and fill out each of the linked forms as they are required before your child’s first day at school.

Primary Enrollment Form (Required - Fill Out)

Emergency Disaster Plan (Required - Read and Acknowledge)
Pesticide Policy (Required - Read and Acknowledge)
Health and Safety Policy (Required - Read and Acknowledge)
Immunization Forms (Required - Submit)
Washington State requires us to have up to date information on your child's vaccinations annually--please submit it with the link below. The CIS (Certificate of Immunization Status) may be printed from the WA Immunization Information System (IIS) or by signing up with MyIRMobile (www.myirmobile.com). If your child's vaccinations are "conditional" or "not complete" you must also sign the Parent Signature section acknowledging that you have 30 days from the date of enrollment to bring your child's vaccinations up to date or provide the necessary exemption paperwork. If a hard copy of the form is used it must be medically verified for accuracy with a health care provider signature.
